Of Chippenham, passed away peacefully on the 2nd February 2022, aged 56 years.
Dearly missed by her beloved husband Stuart, daughters Heather and Caitlin all family and friends.
A Service to Celebrate Hazel's Life will take place at St Mary's Church, Chippenham on Thursday 24th February 2022 at 11.30am.
